Government-issued photo ID with matching address
Lease agreement or mortgage contract with matching address
Current utility bill in your name with matching address

You may attach these at this time. Any carpoolers for whom verification isn't provided will be contacted by MoveMinneapolis to make arrangements for this verification step.
